Do I have to repay forbearance in a lump sum?

Usually no. Most servicers offer repayment plans, deferral of the paused amounts to the end of the loan, or a modification. Ask about the exit options before you accept the forbearance.

Does forbearance hurt my credit?

If arranged before you miss payments, servicers typically report you current under the agreement. Missed payments before the agreement will already have been reported.
